Live Scan Locations in California Fingerprint Cards are required for applicants residing outside of California. The MBC will mail fingerprint cards when the application and processing fees have been received. |

UCSF Fresno has staff members available to provide notary services to residents at no cost. Please contact your program coordinator or the GME office for a current list of notaries at UCSF Fresno. |

Photos |
UCSF Fresno provides photos of current trainees at no charge. Please work with your program coordinator or the GME office to obtain copies from the UCSF Fresno Graphics department. Photos should be head and shoulders only (passport quality), size 2”x2”, full color. Photos are required on application documents before they can be notarized, so please be sure you have photos before you get documents notarized. |

FSMB Online USMLE Transcript Request Applicants should request an official examination history report verifying passage of USMLE Step 3 from the FSMB. Steps 1 and 2 (CK and CS) were reported with PTL application. |
