UCSF Fresno has staff members available to provide notary services to residents at no cost. Please contact your program coordinator or the GME office for a current list of notaries.
UCSF Fresno provides photos at no charge. Please work with your coordinator or the GME office to obtain copies from the Educational Graphic Arts department.
Photos should be head and shoulders only (passport quality), size 2”x2”, full color.
Photos are required on application documents before they can be notarized, so please be sure you have photos before you get documents notarized.

Residents are required to apply for a DEA certificate upon receipt of a license. See the UCSF Fresno DEA policy for details on how to apply and information about fee exemption for your DEA certificate. Fee exemption doesn’t apply for external moonlighting purposes.
